Some Services Offered
Access Community, Social and Civic Activities deliver tailored support to help participants engage with their local communities and build lasting independence. This dual-focused support includes providing dedicated support workers to assist with immediate, hands-on needs—such as accompanying participants to recreational events, facilitating social outings, and assisting with transport—as well as delivering capacity-building programs. Through skill-based coaching, mentoring, and community transition training, providers empower participants to confidently navigate public spaces, develop social networks, and access civic or volunteer opportunities on their own terms.
Assistance with Personal Care Activities deliver essential, one-on-one support to help participants manage their daily self-care routines with safety, comfort, and dignity. Delivered by qualified support workers in the home or community, these vital services include hands-on help or supervision with personal hygiene tasks like bathing, showering, oral care, and grooming, as well as assistance with dressing, toileting, and continence management. Additionally, providers assist with critical physical needs such as safe mobility transfers (like moving from bed to a wheelchair), medication prompting, and mealtime consumption support. By centering care around individual preferences and routines, providers help maintain health outcomes while actively fostering personal autonomy and reducing the physical strain on family caregivers.
Assistance with Personal Domestic Activities deliver practical support to help participants maintain a clean, safe, and organized home environment. Tailored to fit individual routines, this service provides qualified support workers to assist with essential household chores that may be challenging to complete independently. Providers deliver hands-on help with general house cleaning, vacuuming, mopping, laundry, ironing, and changing bed linen, as well as meal planning, grocery shopping, and healthy meal preparation. By keeping the living space sanitary and well-managed, providers ensure participants can enjoy a comfortable and accessible home while freeing up their time and energy to focus on personal goals, hobbies, and community participation.
Providers of House and Yard Maintenance supply essential services to ensure a participant's home remains safe, accessible, and well-kept. Tailored for tasks that a participant cannot reasonably perform themselves due to their disability, this support focuses on keeping both internal and external spaces secure and functional. Providers deliver regular yard care, including lawn mowing, weeding, pruning, and clearing pathways to prevent trip hazards and maintain outdoor accessibility. Additionally, they assist with minor home maintenance tasks, such as replacing light bulbs, smoke alarm batteries, or tap washers, as well as necessary seasonal cleaning like clearing gutters or pressure washing slippery pathways. By managing these heavy or technical chores, providers eliminate safety risks, maintain the integrity of the home environment, and ensure participants can live comfortably and independently.

We also prioritise finding the right personality match for you. Before shifts begin, we look at your goals, interests, and preferences to pair you with a support worker who shares similar energy and hobbies. This ensures you feel completely comfortable, safe, and happy with the team member entering your space.
Connected Community Services operates as an unregistered NDIS provider, working directly with Plan-Managed and Self-Managed participants. Our ultimate goal for Connected Community Services is to eventually become a fully registered provider as we continue to grow.
Because we are a newly opened NDIS provider, we are currently unregistered as we establish ourselves. However, we are definitely not new to NDIS care. Unlike many other unregistered businesses, our team brings four to five years of solid, hands-on experience in the disability sector and NDIS framework.
We take our service quality incredibly serious. We have robust, professional policies and procedures in place that every single team member must follow. Our staff undergo direct, comprehensive training from both our CEO and our Service Coordinator to ensure we are consistently delivering absolutely the highest standard of care.
Every single member of our team is fully vetted, holds current police checks, NDIS Worker Screening clearances, and strictly adheres to the NDIS Code of Conduct. Our focus is entirely on building genuine, trusted connections and delivering premium, reliable support that helps you reach your goals.
